To Apply:  After ordering and provisioning of an EC-3 account, look in the StoreManager for easy access to a Merchant Account Application.  The following information will be needed:

• Federal Tax ID or Corporation or Social Security # (for Sole Proprietor)

• Corporate and DBA Name

• Local Bank Account routing and account numbers

• 3 Trade references

• Average transaction size and monthly volume

• Personal information on principle: DOB, Social Security number, address, phone
